All Families: Please review the Concussion Information and complete the Athlete Registration and Uniform Order Forms.

Athletes 7th-12th Grade: Please also review and complete the OSSAA Physical Evaluation Form.

*OSSAA Physical only required for 7th-12th graders. Physicals are good for 1 year.

Scholarships are available, however funds are limited.
